Personalized nutrition education based on therapeutic and dietary recommendations from the National Institutes of Health and guidelines from the American Dietetic Association

Golden Patient Care Management

Early and on-going collaboration by your interdisciplinary care team addresses your care holistically
Cardiac-specific resources, including your Patient Transition Notebook and cardiac meal plans featuring chef-inspired recipes from Right Rhythm Recipes for Heart Health, give you the tools you need to effectively self-manage your condition at home
Follow-up from your care team 48 hours after discharge and then each week for the next four weeks gives you the opportunity to ask questions about your recovery and gives your care team the opportunity to adjust your plan as needed
